For online client scheduling, click both forms below , fill out, and email as attachments to Craig@HealthAlkemy.com.  The Electronic Systems Survey form can be filled out online.  The intake forms can be filled out on your computer by most people, otherwise they will require you to print and fill out, then scan and send or simply mail to Health Alkemy 700 River Street, Santa Cruz CA 95060.
